Ruabon Medical Centre and all staff members aim to provide you with the highest quality healthcare. To do this we must keep records about your health and any treatment or care which we provide you.

Medical Patient Information and Rights

Your Information

We hold your information on a computer or in a written record, sometimes it’s in both. These records help to guide and manage the care you receive.

This is to make sure that:

  • the people who are involved in your care have accurate and up-to-date information to assess your health and decide what treatment or care you need, and when and where you will receive it. They may be part of the healthcare team or a support service providing your care
  • you are invited to receive routine treatment such as immunisations and relevant screening programmes
  • there is a good basis for assessing the type and quality of care you have received. This will lead to better care both for you and for other patients in the future
  • if you need to complain about the treatment or care you receive, your concerns or complaints can be properly investigated

When we collect and use your personal information, we will ensure this is processed in accordance with at least one of the legal grounds available to us under data protection legislation.

We will process your personal information with your consent or where the law enables us to do so and only when necessary. For example, where we have a legal obligation as a public authority and/or in carrying out our functions or performing a task in the interests of the public.

You may receive care from organisations that are not part of NHS Wales, such as social services or private and voluntary healthcare providers. If so, there may be a need to share some information about you so that everyone involved in your treatment or care can work together for your benefit.

NHS Wales handles your information in the strictest confidence whenever it is used.

We will ensure that:

• only the minimum amount of information needed will be passed on
• anyone receiving information about you is under an obligation to keep it confidential and safe, and to only use the information for the specified purpose(s)
• information sharing agreements between organisations will control the way your information is shared
• secure systems are in place to help prevent unauthorised access to your information

We will keep your personal information for as long as we need to, so that we are able to deliver our services and to make sure that we are providing you with the highest quality care. It will be kept in line with our legal requirements and the law. When your information is no longer required, we will make sure it is disposed of in a secure manner.

Your Rights (Data Protection Laws)

There are laws which provide certain rights to individuals regarding the processing of their personal information.

Within health these rights include, a right to:

  • be informed about the reasons why we collect and use your information. We have a duty to ensure the information we
  • use is limited to what is necessary for that purpose and to either inform you or ask for your consent if we use it for another reason
  • either look at or receive a copy of your health records (whether held in writing or on a computer)
  • correct any inaccurate information we hold on you. We have a duty to keep information about you accurate, however it should be noted that entries in your health record cannot generally be amended, although this will be considered on a case-by-case basis
  • object to us processing your information, for example, for marketing reasons
  • ensure that your information is kept for no longer than is necessary
  • expect your information to be protected from unauthorised or unlawful processing and against accidental loss, destruction or damage

Not all individual rights under data protection law are absolute. Where possible we will look to comply with any request from you, but we may need to hold or process your personal information in connection with one or more of our legal functions.

Freedom of Information Act

The Freedom of Information Act entitles anybody to ask a public authority in England, Wales or Northern Ireland, including government departments, for any recorded information that they keep.

Ruabon Medical Centre is a GP Practice supplying NHS services to it’s patient population within the Ruabon and local area.

Details of meetings with pharmaceutical companies and other medical suppliers:

As a minimum, name of the company, the date and, if appropriate, the name of the member(s) of staff attending (any senior managers and any medically qualified staff), together with a general indication of the category of meeting, for example marketing or promotion.

From time to time we participate in educational lunchtime meetings with pharmaceutical companies to gain training and insight into products available. All our team who are present on those days will attend if possible.

What we spend and how we spend it

We receive a payment for providing medical services to our registered patients from Betsi Cadwaladr University Health Board and Welsh Government.
From this income the practice has to meet the costs of running the practice.
There may be circumstances where material cannot be released because it is:
• confidential or
• commercial information or
• the appropriate officer designated for these purposes under the Act has taken the view that it may be prejudicial to the conduct of the Practice’s affairs or
• no longer available under NHS guidelines and our Practice Records Management Policy.

What our priorities are and how we are doing

Strategies and plans, performance indicators, audits, inspections and reviews:
We are providing a health care service to the patients and population of Ruabon Medical Centre and we undergo rigorous audits and checks. We are reviewed and monitored regularly in regards to performance indicators and have to meet the criteria for Access Standards, GMS contract, Welsh Government Standards, QAIF and QI Framework and Betsi Cadwaladr University Health Board.
Performance data including performance against targets:
We are a fairly high achieving practice and we are monitored and set targets from BCUHB, WG and LMC standards committees.

How we make decisions

All decisions regarding our Practice and the services we provide are documented in the minutes of our Practice meetings which are held at least monthly. Multi-disciplinary meetings are held weekly, fortnightly and monthly. Regular meetings are also held for the nursing and administration staff.

We operate CCTV to provide safety and security to our staff and visitors.

We have a number of measures in place for monitoring and protecting health and safety. These can be discussed upon request and are available for you to read and browse through on our website.

The services we offer

Information about the services we offer, including leaflets, guidance and newsletters produced for the public:
We have an updated website and social media page (Facebook) which we update regularly to keep patients up-to-date and informed of any new services and information. There are two local newsletters produced regularly one in Ruabon and one in Cefn Mawr. Leaflets and information can be obtained from our waiting room at the surgery, via website, social media Facebook page or on request. We have patient information leaflets available in paper format and also for you to download from our website. All forms and information can be downloaded easily.
Out of hours arrangements: During our of hours, you will be able to reach and speak to one of the NHS 111 staff who will discuss, guide and support you to the relevant person for your needs and requests.

Asking for Information

It is preferable that you make your request in writing by letter or email to the address below.

Declining a Request

We will provide the information unless there is a good reason for not doing so. This, for example could be if your request relates to the personal details of another person. If we decline the request, we will set out why. We can be asked to reconsider our decision.

The Cost of Providing the Information

A lot of information may be supplied free of charge. However, sometimes we may have to charge a fee, either to cover administration costs, or to pay photocopying or postage costs, or because of the expense involved in finding and supplying the information.
