Registering at Ruabon

To register as a new patient please call in to the Medical Centre. You will be required to complete the mandatory new patient registration questionnaire and book in for a general health check.  Copies of these forms can be downloaded using the links below.

All new patients will be offered a new patient medical check which will usually be carried out by the centre’s Health Care Assistant. It is important that we gather as much information as possible about your health as this helps our team in making medical decisions about you before we receive your clinical notes from your previous surgery.

Please ask at Reception for a New Patient appointment with the Health Care Assistant when you return the forms to us.  If you are on medication, please bring a recent printed list from your last prescription with you to your appointment.

If you do not wish to attend we should be grateful if you would inform Reception as we are obliged by the Local Health Board to maintain a record.

If you, or any other household members, have moved address or changed your telephone contact details please update the surgery immediately by contacting us via telephone, via email or by completing our contact form.

Ukranian Refugees

We accept Ukranian refugee registrations at the surgery but all patients will firstly need to attend the Catrin Finch Centre to undergo a health screening assessment which is organised and run by Betsi Cadwaladr University Health Board (BCUHB).  All children will need a medical before they will be allowed to attend school.  If you have any queries or need any help and assistance, please contact our Registration Clerk or our Reception Team who shall be able to assist you with any matters you may have in regards to registering and further care and health needs.
